I have given many online exams for various certifications and courses.
There is a person looking at you via the webcam.
They make sure no one else enters the room and that you don't speak with anyone.
You show your id to confirm it is you.
Some companies like Peoplecert even gives you their own software to install that you can't alt-tab to open the internet
There may be ways to cheat, but well... there are always ways to cheat even if not online. I don't see any bigger cheating risk via online proctoring.
